1. Introduction

Welcome to Aria Health Collective ("Aria," "we," "us," or "our"). Aria was founded on the belief that healthcare should treat the whole person, not just a set of symptoms. Our mission — Whole Person Care, Lasting Wellness — guides everything we do, from primary care to mental health support, hormone optimization, and diagnostic testing. We exist to bring real people real support, so our patients can live healthier, fuller lives.

These Terms and Conditions ("Terms") govern your access to and use of Aria Health Collective's services, whether received in person at our Glen Carbon, Illinois location, through our website, or via any patient portal, telehealth platform, or communication channel we operate (collectively, the "Services"). By scheduling an appointment, visiting our facility, creating an account, or otherwise using our Services, you agree to be bound by these Terms.

2. Who We Are and What We Offer

Aria Health Collective is a whole-person health practice offering an integrated range of services, including:

  • Primary Care — routine and preventive health services

  • Mental Health — behavioral and emotional wellness support

  • Hormone Optimization — assessment and management of hormonal health

  • Testing & Wellness — diagnostic testing and ongoing wellness support

Our goal is to bring these services together in one place so patients experience coordinated, continuous care rather than fragmented visits across unconnected providers.

3. Eligibility

You must be at least 18 years old, or the legal age of majority in your jurisdiction, to independently schedule and consent to Services. Care for minors requires the consent and involvement of a parent or legal guardian in accordance with applicable law.

4. Medical Disclaimer

Aria Health Collective provides individualized clinical care delivered by licensed professionals. However:

  • Any general health information provided through our website, marketing materials, or educational content is for informational purposes only and is not a substitute for personalized medical advice, diagnosis, or treatment.

  • Always seek the advice of your Aria provider or another qualified health professional with any questions regarding a medical condition.

  • If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not rely on our website, portal, or non-urgent communication channels for emergency needs.

5. Appointments, Cancellations, and No-Shows

  • Appointments may be scheduled by phone, in person, or through our online systems where available.

  • We request that you notify us at least [24/48] hours in advance if you need to cancel or reschedule.

  • Late cancellations or missed appointments ("no-shows") may be subject to a fee, at Aria's discretion, as communicated at the time of booking.

  • Aria reserves the right to decline to schedule or to discharge a patient from care where appropriate and consistent with applicable law and standards of medical ethics, with reasonable notice and assistance in transitioning care where required.

6. Fees, Insurance, and Payment

  • Fees for services vary by the type of care provided and will be communicated to you prior to or at the time of service where practicable.

  • You are responsible for understanding your insurance coverage, if applicable, including co-pays, deductibles, and any services that may not be covered.

  • Payment is due at the time of service unless other arrangements have been made in advance with our billing team.

  • Aria reserves the right to update pricing for services from time to time.

7. Privacy and Protected Health Information

Your privacy matters to us. Our collection, use, and disclosure of your protected health information ("PHI") is governed by our Notice of Privacy Practices, provided separately in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable Illinois law. By receiving care from Aria, you acknowledge that you have been offered a copy of that Notice.

8. Patient Portal and Electronic Communications

If Aria offers a patient portal, telehealth platform, or other electronic communication tool:

  • You are responsible for maintaining the confidentiality of your login credentials.

  • You agree to provide accurate, current information when using these tools.

  • Standard email and text messaging are not fully secure; please use the designated secure portal for sharing sensitive health information whenever possible.

  • These tools are not intended for urgent or emergency communications.

10. Limitation of Liability

To the fullest extent permitted by applicable law, Aria Health Collective and its providers, officers, and staff shall not be liable for indirect, incidental, or consequential damages arising from your use of our Services, except where such liability arises from professional negligence in the provision of clinical care, which remains governed by applicable medical malpractice and healthcare liability law. Nothing in these Terms limits any liability that cannot be excluded or limited under applicable law.

Aria may update these Terms from time to time to reflect changes in our practices, services, or legal requirements.
